The Middle Island Dog Park will be a Membership Only Park.

An Attendant is on Duty From Opening Until Closing, 7 Days Per Week.

Each park patron will be issued his own membership card, aka "Pooch Pass," plus a corresponding tag to be worn by the pup at the park. This Pooch Pass will be shown each time to the attendant on duty at the park, and anyone not having the Pooch Pass will be denied entry.

The annual fees for the Pooch Pass are:

---Brookhaven town residents---  $25

---Non resident of Brookhaven Town--- $45

---Seniour citizen resident---$10

---Seniour citizen non/resident---$45

---Replacement of lost/damaged passes- $5

---Amended pup owner pass (ie, adding a new pup to the family) --- $5

---Donation to pup park (voluntary)--- $1

Application Requirements for Membership---

Please note: beginning November 19, applications for the Pooch Pass will be processed at the Town Clerk's Office, Brookhaven Town Hall, Bicycle Path, Farmingville, and will be valid from official date of opening for a period of one year.

---PUPS MUST BE SPAYED OR NEUTERED, as no intact males or unspayed females will be allowed in the park

---CURRENT BROOKHAVEN TOWN DOG LICENSE- if your pup  is not currently licensed, please license expeditiously at the Town Clerk's Office, Town Hall Farmingville, phone 451-9101, or at the Brookhaven Town Animal Shelter, 300 Horseblock Road, Brookhaven, phone 286-4940. You must bring proof of current rabies vaccs and proof of spay/neuter for this license. The fee is $2.50

---You need to bring to the town clerk's office the following: your driver's license, your NYS dog license, proof of current rabies vaccination, proof of spay/neuter, and a picture of your pup- do not cut it, they will size it to fit the pass for you.

---Minimum age of Dog Owner (applicant) will be 18 years old

---Applicant (dog owner) will be required to show their driver's license (or a non driver ID card) for photo ID purposes

---Applicant must be the dog owner of record ( as stated on their pup's NY State dog license)

---Applicant must sign an agreement on the application to follow the rules of the pup park or their pass will be permanently revoked

---Pups must have their own Pooch Pass

---Applicant (dog owner) must submit a small photo measuring 2X2 of each pup to be superimposed on the pooch pass

---Each pup will be screened against the Dangerous Dog Database.  No pass will be issued if the pup is listed in the database.

---The Pooch Pass info will tie in with the Dog Owner's Pass with a unique permit number.

---The Pooch Pass will be permanently revoked if the pup is later court-determined to be a dangerous pup

---The Pooch Pass will include the name, sex, breed of pup, colour, dog license number, a unique permit number and the spay/neuter status of pup

---No refunds will be issued

---The Pooch Pass and the Dog Owner Pass will be colour coded by year
